Personal Information Protection and Electronics Documents Act
For swimmers under the age of 18, this form must be completed and consent provided by a parent or legal guardian. Swimmers who are 18 years of age or older may provide consent on their own behalf.
The federal Personal Information Protection and Electronic Documents Act (PIPEDA), and applicable provincial privacy legislation, requires organizations to obtain appropriate consent for the collection, use and disclosure of personal information.
Personal information provided to the Peterborough Swim Club through this registration will be collected, used and disclosed for purposes reasonably associated with the swimming activities and operations of the Club. These purposes may include:
- Registration with Swimming Canada, Swim Ontario and applicable events;
- Administration of insurance coverage;
- Training, competition and participation in swimming activities;
- Establishing swimmer eligibility and participation in an age-appropriate training and competition environment;
- Establishing athlete eligibility for selection to swim teams and other programs;
- Maintaining pertinent medical and emergency information;
- Reporting non-identifying demographic and participation statistics to funders, sponsors and other authorized third parties;
- Publishing swimmer names, gender, age and club affiliation on Club or association websites, competition results, news releases and ranking reports; and
- Communicating directly with swimmers and/or their parent or legal guardian, as applicable, regarding the operation of the Club, Swim Ontario and Swimming Canada.
Some of the personal information provided through registration may be shared with Swimming Canada and Swim Ontario for purposes including association registration, insurance coverage, athlete eligibility, competition participation, record keeping and other purposes reasonably associated with the administration of swimming programs.
Additional personal information may be collected from time to time. Where the purpose for collecting and using the information is obvious and the information is voluntarily provided, consent may be inferred. Where the purpose is not obvious, the purpose for collection will be provided before or at the time the information is collected, either orally or in writing.

A swimmer may request access to their personal information held by the Peterborough Swim Club, Swim Ontario or Swimming Canada by submitting a request to the appropriate organization in accordance with that organization's privacy policy. Consent to the collection, use or disclosure of personal information may also be withdrawn in accordance with the applicable privacy policy.
I understand that withdrawing consent may affect the ability of the Peterborough Swim Club, Swim Ontario or Swimming Canada to provide certain services, administer registration, insurance, training or competition activities, and may result in the cancellation of registration or suspension of participation where the information is necessary for those activities.
This consent must be provided each season as part of the registration process.
